This form only accepts image/gif or image/jpeg or image/pjpeg files
Restrictions are 300k ,1000pix by 1000pix
Please rename your files with your last name, page they should go on, and a number.
For example:smith_orderpage_pic2.jpg
Please email us to tell us you have uploaded images!
You can paste your text into an e-mail, or send it as a Word .doc (if you want us to see the formatting) or .txt file. You can also send your images as attachments or compress them into a ZIP file and e-mail them.
For large amounts of text and/or images, you might want to send regular photos, or put them on a CD, and surface mail them to us.
